椎基底动脉延长扩张症(VBD)相关脑及神经疾患是后颅窝异常血管压迫引发的神经血管压迫综合征,曾因临床认知不足、诊断缺乏统一标准、治疗策略针对性不强等问题,成为神经外科诊疗难点。2025年相关诊疗规范团体标准的发布实现了该疾病诊疗相关内容的系统性归纳。基于该标准核心内容及多中心临床循证证据,本文从VBD的病理机制、诊断体系构建、治疗策略优化及未来发展方向等方面展开述评,以期为该疾病的规范化与精准化诊疗提供参考。

Vertebrobasilar dolichoectasia (VBD)-related cerebral and neurological disorders, a special neurovascular compression syndrome caused by abnormal vertebrobasilar artery compressing adjacent brain tissues and cranial nerves, have long been a clinical challenge due to insufficient cognition and lack of unified diagnostic criteria. Due to insufficient cognition, lack of unified diagnostic criteria, and weak targeted treatment strategies, this condition has long posed challenges in neurosurgical diagnosis and management. The release of the relevant diagnostic and therapeutic standardization group standard in 2025 has systematically summarized diagnostic and therapeutic content for this disease. Based on the core elements of this standard and multicenter clinical evidence-based research, this article reviews the pathological mechanisms of VBD, the development of diagnostic systems, optimization of treatment strategies, and future research directions, aiming to provide a reference for standardized and precise diagnosis and treatment of this condition.
