Postoperative complications and influencing factors after cranioplasty

目的

探讨颅骨修补术后并发症的相关影响因素并进行临床分析。

方法

选取联勤保障部队第九四医院神经外科自2015年3月至2020年5月诊治的203例颅骨修补患者的临床资料,收集可能与患者颅骨修补术后并发症存在相关性的因素,采用单因素分析和多因素Logistic回归分析相关因素与患者并发症的关系。

结果

术后44例患者出现并发症,其中感染6例,继发癫痫15例,钛网暴露9例,颅内出血7例,硬膜外积液6例,死亡1例。通过单因素和多因素Logistic回归分析发现年龄(>50岁)、颅骨缺损面积(>10 cm×10 cm)及修补时间(>6个月)为患者颅骨修补术后并发症出现的独立危险因素(P<0.05)。

结论

患者年龄、颅骨缺损面积及修补时间是颅骨修补术后并发症发生的独立危险因素,重点评估上述指标并严格规范操作,争取早期行颅骨修补,重视其全身疾病,可以有效减少并发症的发生。

Objective

To explore the related factors of complications of cranioplasty and make clinical analysis.

Methods

The clinical data of 203 patients with cranioplasty treated in Neurosurgery Department of 940th Hospital of the Joint Service Support Force from March 2015 to May 2020 were collected. The factors that may be associated with postoperative complications of cranioplasty. Univariate analysis and multivariate Logistic regression were used to analyze the relationship between related factors and complications.

Results

Postoperative complications occurred in 44 patients, including 6 cases of infection, 15 cases of secondary epilepsy, 9 cases of titanium mesh exposure, 7 cases of intracranial hemorrhage, 6 cases of epidural effusion, and 1 case of death. Univariate and multivariate Logistic regression analysis showed that age (>50 years old), skull defect area (>10 cm×10 cm) and repair time (>6 months) were independent risk factors of complications after cranioplasty (P<0.05).

Conclusion

Patients' age, skull defect area and repair time are independent risk factors for complications after cranioplasty. It can effectively reduce the occurrence of complications by focusing on the evaluation of the above indicators and strictly standardizing the operation, striving for early cranioplasty and paying attention to their systemic diseases.
