目的

探讨半-半椎板入路显微手术切除C2段脊神经鞘瘤的手术效果和安全性。

方法

回顾性分析山东第一医科大学第一附属医院神经外科自2014年6月至2018年6月收治的12例C2段脊神经鞘瘤患者,均采用半-半椎板入路手术治疗,对手术前后临床资料进行分析,评价患者的临床疗效及脊柱稳定性。

结果

所有12例肿瘤均获得全切,术后病理证实均为神经鞘瘤,其中包括8例为哑铃型肿瘤,4例为小型球型肿瘤,术后及随访期内(6~36个月)患者术前症状明显改善,无手术并发症、肿瘤复发及脊柱不稳发生。

结论

半-半椎板入路手术能最大程度地保留原有骨性结构及脊柱稳定性,为C2段脊神经鞘瘤的切除提供足够的空间,可作为一种优先选择的手术方案。

Objective

To investigate the effect and safety of microsurgical resection of spinal neurilemmoma of C2 segment through hemi-semi lamina approach.

Methods

Twelve patients with spinal neurinoma of C2 segment who were treated in the First Affiliated Hospital of Shandong First Medical University from June 2014 to June 2018 were analyzed retrospectively. They were all treated by hemi-semi lamina approach. The clinical data before and after the operation were analyzed to evaluate the clinical efficacy and spinal stability of the patients.

Results

All the 12 tumors were completely resected, all of which were confirmed by postoperative pathology as neurilemmoma, including 8 cases of dumbbell tumor, 4 cases of small spherical tumor. The preoperative symptoms of the patients were significantly improved after the operation and during the follow-up period (6-36 months), and there were no surgical complications, tumor recurrence and spinal instability.

Conclusion

Hemi-semi lamina approach can retain the original bone structure and spinal stability to the greatest extent, and provide enough space for the resection of C2 neurilemmoma. It can be used as a preferred surgical option.
